Youth Reference Check

This form allows youth applicants to volunteer and serve throughout Salem Community Church before turning 18 years old. All youth applicants must provide a reference completed by a non-relative. The reference should be someone who can speak to the applicant’s character, responsibility, and ability to serve well in a ministry setting.
